Does the middle mouse button do anything over step, next,
and so forth?  Perhaps I have missed this suggestion, but
how about a toggle that can be used when the mouse is over
these buttons.  As usual, the little window in the upper
righthand corner will note that the middle mouse button can
toggle between making this a "stuck" selection (maybe both
"step" and "next") and not, and basically typing space or
escape or "s" will cause a step to occur.

This has the benefit of associating what's going on with
the action used to invoke it, and it's less likely to cause
confusion due to accidental activation.  One could even
grey out the typing area, other keys, as can be done now,
to drive home the point that you're in a special mode.

My $0.015 (because I haven't been following the conversation
closely enough to know whether this had been suggested).
